- Your name, address, phone number, and email address.
- A statement that you consent to the jurisdiction of the federal court in your district (or the district in which the service provider is located).
- A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.
- Identification of the material that has been removed and the original location of the material before it was removed.
- Your physical or electronic signature.

3. Repeat Infringer Policy
FriendlyWell.com reserves the right to terminate, in appropriate circumstances, users or account holders who are repeat infringers of intellectual property rights.
4. Misrepresentation
Under Section 512(f) of the DMCA, any person who knowingly misrepresents that material or activity is infringing may be subject to liability. Please ensure you are the legal copyright holder or authorized to act on behalf of the copyright owner before filing a DMCA notice.
